Tips For Collecting & Sending Your Records

- Clear

Check your scanned pictures or photographs are very clear.

- Complete

Don't forget to scan front and back of each page if there is any information on them,

whether it seems relevant or not.

- Identify

If you are sending records for more than one person,

make sure the name of the person is clear on each page.

 

- Your GP

Contact all the GP clinics you may have had vaccines at and ask

for your immunisation history at that clinic to be emailed to you, or printed for you.

(Due to the Privacy Act, we cannot do this for you.)

 

- Check Everywhere

Don't forget you may have had vaccines at more than one GP clinic or a travel health clinic. 

You will need your record from each of these places.

If you have a My Health Record, your vaccination records may have been stored there too.
